About Us?

The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.

The Integrated Team Care (ITC) program aim is to improve access to the most appropriate health services for Aboriginal and Torres Strait Islander people at the right time and in the right place with a view to reducing avoidable acute hospital admissions or readmissions and emergency department presentations; improved use of the healthcare system and to support people to take part in a healthy lifestyle.

About the Role?

The purpose of the position is to provide support to mainstream general practice to improve access to private general practice for Indigenous people. This will include working directly with a defined number of mainstream general practices in the familiarisation of CTG initiatives; and supporting mainstream general practices in the provision of appropriate and timely care for Indigenous people in their region, especially those at risk of or who already have a chronic disease.

Day to Day duties include:

  • Lead the periodic monitoring and review of new strategic geographic target areas by the CTG Team; including analyse demographics of indigenous population in Metro South region on a ‘no compete’ basis with AICCHS client catchment areas.
  • Allocate geographic and GP Practices targets to Project Officer and IOWs, as required.
  • Analyse own target Partner Practices and develop a Report that analyses the existing systems, procedures and technologies in each Practice; analyses baseline client data; identifies value-add benefits the IUIH/CTG Team can contribute and identifies Care Coordinator and IOW value-adding roles.
  • Coordinate other IUIH programs/staff engagement with partner Practices.
  • Assist each partner practise to implement valid self-identification procedures for Aboriginal and Torres Strait Islander people.

  • Current C Class Drivers Licence (Qld)
  • Current Criminal History Check

Desirable, but not mandatory:

  • Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
