Our client has been a highly respected and trusted provider of aged care, disability, child and family services across the mid-north Coast. Due to ongoing growth we currently have an opportunity for an experienced Coordinator of Supports to join their team in Macksville. The position of a Support Coordinator is to provide individualised support to clients of the National Disability Insurance Scheme (NDIS). The Support Coordinator will work with the clients to implement the supports within the persons plan for the purpose of building capacity, managing the impact of their disability and improving their health, well-being and participation within the community.
Key responsibilities:
- Provide support to clients based on their NDIA plan, implementing all supports within the clients plan including formal, mainstream and funded supports.
- Assist the client to understand the supports in their plan, access and use the client portal, search for contact and select providers.
- Co-design individualised plans and source support programs with clients, carers and relevant stakeholders that will meet the agreed outcomes of their (costed) care plan.
- Manage client issues and complaints with respect and fairness whilst following policies and procedures.
- Prepare the client to think about their next plan and review their goals by evaluating their current supports, as well as exploring new supports.
- Support the client and their network to understand how to client in the NDIS processes, such as establishing agreements with service providers, managing budgets flexibility, and setting and refining goals, objectives and strategies.
- Ensure regular contact is maintained and recorded.
- Maintain a working log of all contacts with clients, families/carers and support services within a database (SupportAbility).
- Monitoring reports & documentation plus quality reports for internal committees as required.
- Bill for all supports provided by rostering in SupportAbility to ensure payment for supports delivered
- Prioritise, manage and administer a caseload / tasks in a coordinated, efficient and timely manner.
- Ensure that information on relevant community resources are available to clients and that appropriate referrals are made and followed up
What we’re looking for:
- A minimum five years working in disability services or a similar environment with demonstrated experience in complex disability support
- Relevant tertiary qualification in; Disability, Individual Support, Community Services, or substantial demonstrated experience
- Comprehensive knowledge of human rights-based approaches and person-centred principles and approaches, including positive behaviour support and active support
- Ability to manage case load of 20-30 clients
- Ability to coach, mentor and develop teams
- Well-developed written, verbal and listening skills
- Capacity to implement change and identify opportunities for innovation
- Solid financial management capability (billable hours, P&L’s, budgets)
- Confidence and capability in using systems and technology
- Strong administrative skills: time management, coordination of tasks, efficient work practices
- Ability to plan and schedule own work independently, monitor progress against work plans and outcome
- Excellent interpersonal skills to engage clients, their family/guardians/carers, advocates and all other key stakeholders
- Current Working with Children Check – DWES - National Police Check
