At Transition Life Care, we are a values-driven NDIS provider committed to delivering trauma-informed, person-centred support. Our mission is to walk alongside participants as they navigate the NDIS, build capacity, and live meaningful, empowered lives.
We are seeking a Support Coordinator / Psychosocial Recovery Coach to join our team. This dual role blends practical coordination with recovery-oriented coaching to support individuals with psychosocial disability in achieving their goals and strengthening independence.
Already supporting participants? We warmly welcome applicants with an existing caseload looking for a supportive, collaborative home.
Your Role

Connect participants with appropriate services, including community, mainstream, clinical, and informal supports.
Coordinate complex supports and advocate for participant needs in a professional and trauma-informed manner.
Complete accurate documentation, progress notes, and reporting—including formal Recovery Plans submitted to the NDIS.
Participate in networking and community engagement to help grow our presence across the Gold Coast, Tweed, and Northern Rivers.
What We're Looking For

Solid understanding of the NDIS framework and psychosocial disability supports.
Highly organised with strong budgeting, problem-solving, and communication skills.
Commitment to trauma-informed, person-centred, and culturally sensitive practice.
Relevant qualification in Mental Health, Social Work, Psychology, Occupational Therapy or related field
Empathetic, reliable, and skilled at maintaining boundaries while fostering empowerment
Why Join Transition Life Care?
A supportive, ethical, trauma-informed workplace.
Flexible working conditions, including work-from-home options.
Competitive salary and genuine work-life balance.
Ongoing professional development, supervision, and growth pathways.
The opportunity to make a meaningful impact while helping shape the future of a growing organisation.
