Role Overview
We are seeking a highly skilled and dedicated Specialist Support Coordinator to join our growing team. This pivotal role involves providing intensive, time-limited support to NDIS participants with complex support needs and challenging circumstances. The Specialist Support Coordinator will work collaboratively with participants, their families, and a wide range of service providers to navigate the NDIS, build capacity, and achieve their stated goals.
This position demands a deep understanding of complex support systems, strong problem-solving abilities, and a commitment to person-centred practice. The successful candidate will play a critical role in empowering participants to exercise choice and control over their NDIS plans, ensuring the effective implementation of supports, and facilitating positive outcomes in their lives.
Qualifications & experience
Minimum 2 years experience in Level 3 or Level 2 Support Coordination experience , NDIS Planner or LAC Role, or similar role with proven excellent understanding of support co-ordinators role and NDIS sector; or an explanation of lived experience and how this relates to the role.
Required for the role is Relevant Tertiary qualifications (Specialist Support Coordinator) or experience in one or more from the following: Support Coordination, LAC, NDIA Planner, Disability Support Worker, Mental Health Worker, Welfare Worker, Social Worker or Allied Health Professional.
Ability to directly service participants living in South West Victoria areas of Warrnambool, Hamilton, Terang, Portland, Timboon, Camperdown and Port Fairy.
Provision of workers clearances, eg. National Police Certificate, WWCC, NDIS workers Clearance
Current COVID-19 vaccinations
Drivers licence insured, reliable car (preferable, not essential)
Strong communication and organisational skills
Honest and transparent and willing to access external supervision if requested
Tasks & responsibilities
Supporting participants to become more independent and encouraging them to achieve their goals.
Focus on positive outcomes
Prioritise, manage and administer a caseload/tasks in a coordinated, efficient and timely manner
Bill for all supports provided to ensure payment for supports delivered
Develop and maintain effective working relationships with other Disability services and mental Health services, other non-government agencies and the NDIA
Ensure that information on relevant community resources is available to clients and that appropriate referrals are made and followed up.
Maintain and monitor client budgets within NDIS funding guidelines.
