ABOUT THE ROLE
As the Support Coordinator, you will manage a caseload of NDIS participants, providing quality support coordination services to assist participants in understanding their plans, navigating their options, and connecting and engaging service providers. You support the participants to work towards their personal goals and live more independently. You are the key point of contact, supporting informed decision-making and choices based on strong relationships.
KEY RESPONSIBILITIES
- Work closely with people with psychosocial disability, their carers, families, and support networks to identify needs, access supports, and achieve their NDIS goals.
- Use a strengths-based, person-centred approach to build participants’ capacity for self-direction, community participation, and independent decision-making.
- Establish and manage Service Agreements and service bookings, ensuring efficient administration through the NDIS portal.
- Connect participants with appropriate services, community supports, and meaningful activities, while fostering strong relationships with both registered and unregistered providers.
- Monitor and manage NDIS budgets, ensuring appropriate use of funds, accurate billing, and achievement of utilisation and financial targets.
- Maintain accurate and up-to-date case records and billing information.
- Support participants through plan reviews by preparing documentation, tracking outcomes, and submitting timely NDIA progress reports.
- Collaborate with the NDIA, Local Area Coordinators, service providers, and mainstream services to deliver coordinated, person-centred support.
- Ensure all services are culturally appropriate, accessible, and respectful, using translated materials and inclusive practices.
- Maintain established case management systems and reporting processes.
QUALIFICATIONS + EXPERIENCE
Essential
- Experience coordinating care for people with people with psychosocial disability, applying recovery-oriented, trauma-informed and person-centred approaches
- Experience advocating within NDIS and other government policy and legal frameworks
- Experience managing and maximizing utilization of NDIS budgets
- Experience building participant capacity to understand and implement their own supports
- Experience working towards and achieving KPIs
- Proficient in MS Office and databases
Desirable
- Experience as an NDIS Support Coordinator
- Qualification in Community Services, Mental Health, Allied Health, Social Work, or other relevant field
- Knowledge of Sydney-based support services.
