Position Summary
As a Support Coordinator, you will assist NDIS participants in navigating and implementing their plans. Your role will involve identifying and coordinating supports and services that align with participants’ goals, ensuring they receive optimal care and assistance.
Key Responsibilities
Provide advanced support coordination to NDIS participants, focusing on complex and high-needs cases.
Develop and maintain strong relationships with participants, families, service providers, and other stakeholders.
Assist participants in understanding their NDIS plans, rights, and opportunities.
Coordinate and monitor funded supports and services to ensure their effectiveness and alignment with participants’ goals.
Manage crisis situations and ensure participants receive timely and appropriate interventions.
Prepare detailed reports, case notes, and updates for participants and stakeholders as required.
Stay updated on NDIS guidelines, legislation, and best practices to ensure compliance and quality service delivery.
Qualifications and Skills
Essential:
Extensive experience in NDIS support coordination, particularly with complex cases.
In-depth knowledge of the NDIS framework, policies, and processes.
Relevant tertiary qualifications in Social Work, Psychology, Disability Services, or a related field.
Exceptional communication, negotiation, and interpersonal skills.
Strong problem-solving abilities and crisis management experience.
Ability to work independently, manage time effectively, and meet deadlines.
Proficiency in using case management systems and technology to support remote work.
Desirable:
Experience working with culturally and linguistically diverse populations.
Knowledge of community resources and support networks in Sydney.
Previous experience in a remote or work-from-home role.
