📍 Townsville LGA, QLD – Remote Start + Local Travel | Contractor or Full-Time

About the Role

Irena Care is a registered NDIS provider expanding into thin-market regions like Townsville, where participants face challenges accessing consistent, culturally safe, and high-quality support.

We are seeking a locally embedded professional to represent Irena Care in the Townsville LGA and surrounding areas. In this role, you will:

Acquire and onboard NDIS participants through community connections

Deliver support coordination to participant with varied and complex needs

Engage with stakeholders and serve as the local face of Irena Care

You can join us through a flexible 3-month ABN contractor agreement, or transition immediately into a full-time role if you bring an existing caseload.

The role starts working remotely from home, with a future transition to a local office setup as regional operations grow.

Key Responsibilities

Participant Acquisition & Intake

Build referral pipelines with GPs, hospitals, LACs, ACCHOs, PHNs, and allied health providers

Represent Irena Care at expos, forums, and community events in Townsville

Oversee compliant participant intake and onboarding, including service agreements and documentation

Work with the central intake and clinical teams to support complex participant transitions

Support Coordination (Level 2 or Specialist)

Manage a caseload of participants, including those with SIL, psychosocial, and complex support needs

Assist participants in understanding and implementing their NDIS plans

Coordinate services, facilitate reviews, lead crisis response, and advocate on behalf of participants

Maintain accurate documentation and submit timely reports via CRM

Community & Stakeholder Engagement

Build strong, ongoing relationships with local providers and referrers

Identify regional service gaps and collaborate on solutions with leadership

Represent Irena Care with professionalism and cultural sensitivity at all times

Be a visible leader in Townsville’s support ecosystem

Who You Are

You may come from:

NDIS Support Coordination

Allied Health, Aged Care, Mental Health, Justice, Youth Work, or Community Case Management

You have strong local networks in or around Townsville

You’re a self-starter, relationship-builder, and strong communicator

You’re passionate about outcomes and community empowerment

You’re ready to lead your region with purpose

Essentials:

Valid Driver’s Licence + Reliable Vehicle (fuel card provided)

Ability to work independently and meet participant engagement KPIs

Prior NDIS experience is welcome but not essential—full support and onboarding provided
