About the Role

668 Care, a nationally registered NDIS provider, is expanding into underserved regions across Queensland where participants often lack access to consistent, culturally safe, and high-quality support.

We are seeking a community-connected leader based in or near Bundaberg, Hervey Bay, Maryborough, Childers, or surrounding Wide Bay–Burnett towns to lead participant engagement and service coordination in the region.

As our Community Partnerships & Senior Support Coordinator, you will:

Acquire and onboard NDIS participants through trusted relationships

Deliver Level 2 and/or Specialist Support Coordination for diverse, complex needs

Represent 668 Care across community networks and service systems

You can begin with a flexible 3-month contractor agreement (ABN) or convert immediately into a full-time role if you bring an active caseload. The role starts remotely from home, with local travel supported by our back-office team.

Key Responsibilities

Participant Acquisition & Intake

Build referral pipelines with GPs, hospitals, LACs, PHNs, ACCHOs, and allied health

Represent 668 Care at community expos, forums, and interagency meetings

Oversee compliant onboarding: intake forms, service agreements, and risk assessments

Collaborate with our central intake and clinical teams for smooth participant transitions

Support Coordination (Level 2 or Specialist)

Manage a caseload across Bundaberg and the Wide Bay–Burnett region

Implement NDIS plans, coordinate services, and provide crisis support

Navigate SIL, psychosocial, and thin-market challenges

Maintain accurate records and service notes using our CRM system

Community & Stakeholder Engagement

Establish and grow partnerships with referrers, NGOs, councils, and local services

Identify service gaps and contribute feedback to the leadership team

Represent 668 Care with professionalism, cultural sensitivity, and reliability

Be a visible and trusted presence in your local care ecosystem

Who You Are

You may come from:

Support Coordination, Local Area Coordination (LAC), Allied Health

Mental Health, Aged Care, Youth Work, Justice, or Community Services

You bring:

Strong local knowledge and deep community ties in Wide Bay–Burnett

Experience working with diverse, rural, or First Nations participants

Confidence working independently with a values-aligned provider

A passion for achieving strong outcomes in the disability sector

Essentials:

QLD Driver’s Licence + Reliable vehicle (fuel allowance provided)

Comfortable managing remote admin and stakeholder travel

NDIS Worker Screening + Police Check (or willingness to obtain)
