JOB OVERVIEW:
The Support Coordinator will assist clients to build their capacity to make informed decisions and choices about the implementation of all supports in their NDIS plan. In addition to supporting access to other mainstream and community funded supports with a focus on overcoming barriers to achieve goals. The Support Coordinator will be expected to take a strength based and person-centred approach with each client adapting their communication style and approach to match client preferences to support reaching the best outcomes in partnership with the client. We are a NDIS registered provider of Support Coordination and Psychosocial Recovery Coaching and pride ourselves on a high standard of service and outcomes for our clients.

WHAT THIS JOB WANTS FROM YOU:
Experience, Skills & Qualifications:
Understanding and appreciation of the needs of the LGBTIQ+ and neurodiverse communities
Demonstrated understanding of NDIS funding, individualised funding approach and NDIS frameworks
Relevant tertiary qualification (degree or Diploma in Community, Social or Human Services) with a minimum of 3+ years relevant experience, NDIS case management preferred.
NDIS Worker Screening and completion of NDIS orientation modules
Advanced skills in Microsoft Office skills (Word, Excel, PowerPoint, and Outlook)
Demonstrated interpersonal skills inclusive of verbal and written communication skills.
Time management skills and ability to prioritise tasks.
Current Driver’s License and suitable car and appropriate comprehensive and roadside assistance insurances
Key Duties:
Accountable for management of assigned caseload of clients
Case noting in a timely and accurate manner to ensure up to date client records are kept, client is invoiced appropriately.
Client documentation and files are kept up to date and consistent with internal requirements.
Able to work autonomously to self-manage and prioritize case load and work tasks under high pressure environment with competing demands to meet client and business demands.
Ability to effectively monitor client budgets and support projections as requested
Provision of home office equipment and environment inducive of productive and ergonomically correct working environment
It is an expectation of this position that they are up to date with the latest trends and information regarding funding so research and reading latest NDIS price guides and funding guidelines
