Connect4U Australia is an NDIS-registered, independent provider of Level 2 and Level 3 support coordination and psychosocial recovery coaching. We are seeking two enthusiastic, outcome-focused Support Coordinators / Psychosocial Recovery Coaches (subcontractor position) to join our team. Please only apply if you live or are willing to travel to the Brisbane North and or Sunshine Coast region on a regular basis.

Support Coordinator / Recovery Coaching experience or human services case management experience is essential. Qualifications and experience with psychosocial disability are essential. Lived experience is also preferable. The successful applicant must live in the relevant geographical area and have good local knowledge and connections.

The Support Coordination role assists Participants to:

Implement their plan and increase their capacity to manage/direct their own supports.

Have greater opportunities to explore and connect with their community and alternative support options.

Strengthen their informal support networks.

Maximise the value for money they receive from their supports.

Access and use the NDIS Participant Portal.

Effectively communicate and maintain relationships with the NDIA.

Build capacity for participants to understand their plans and the different rules that NDIS has on spending their funding

The Psychosocial Recovery Coach role:

Develops recovery-enabled relationships, with a trauma informed approach

Supports NDIS participants with recovery planning

Coaches NDIS participants to increase recovery skills and personal capacity, including motivation, strengths, resilience and decision-making

Supports NDIS participants to engage with the NDIS

The successful applicants will have a caseload of Participants who have support coordination and /or psychosocial recovery coaching funding. These positions offer work/life balance with work from home, and flexible hours to be completed on a full-time equivalent basis. Support Coordinators / Psychosocial Recovery Coaches are mentored by dedicated Managers who are experienced practitioners in the support coordination field. Successful applicants who excel in the role have the potential to increase their pay rate and earn over 6 figures annually.
