We are looking for a Support Coordinator/Specialist Support Coordinator in Brisbane, Ipswich, Sunshine Coast, Gold Coast or Toowoomba region.

More recently we have commenced providing Support Coordination and Specialist Support Coordination.
About The Role
As Emotional Balance’s Support Coordinator/Specialist Support Coordinator, you will be responsible for delivering high-level and professional information for the crucial role of implementing a person’s NDIS Plan, connecting the person with the right supports to build their independence and resilience.
This role requires working with individuals with a disability or complex mental health difficulties.
Key Responsibilities:
Support NDIS participants and their key supporters to achieve their goals.
Conduct risk assessments and address complex barriers.
Design and implement person-centered plans tailored to participants’ needs.
Promote and protect participants’ safety and build their capability.
Collaborate with participants and stakeholders to implement NDIS plans and connect with appropriate supports.
Monitor and manage participant plans, ensuring services are within budget.
Develop strong relationships with participants, families, and providers to drive positive outcomes.
Manage a caseload of NDIS participants, maintaining compliance with NDIS guidelines.
Provide resources to stakeholders to enhance participant support.
What you will bring to the role:
If you are applying for the role of Specialist Support Coordinator, you will:
Have tertiary qualifications in Psychology, Social Work, a Bachelor's Degree related to human services, or a Diploma in Community Services. You will also have NDIS knowledge and experience.
OR you will have extensive experience in the field as a specialist support coordinator
If you are applying for the role of Support Coordinator, you will:
Preferably have tertiary qualifications as listed above. You will also have NDIS knowledge and experience.
Preferably have experience as a support coordinator
OR, currently working in the disability sector.
Be organised and have excellent time management to complete the necessary paperwork and reporting;
Advanced communication skills that are supportive, demonstrates integrity, compassion and inclusion;
Be able to connect and build meaningful relationships with a range of people from all walks of life;
Be innovative and collaborative with highly developed problem-solving skills;
You will be confident in navigating computer and IT software;
Have your own form of transport and a valid driver’s license; and
You have all the necessary paperwork to evidence rights to work.
