Summary:
We’re looking for a compassionate Support Coordinator to help participants navigate their NDIS plans, build capacity, and access the supports they need. This full-time role is responsible for providing high-quality, person-centred support to NDIS participants - helping them understand their NDIS plan, connect with appropriate services, and work toward their goals.
This position offers a pathway to becoming a Specialist Support Coordinator for the right person, with tailored professional development and mentoring available. It’s an exciting opportunity to join a supportive and collaborative team that values your contribution and growth.
Responsibilities:
- Supporting participants to understand and implement their NDIS plans
- Building participants’ confidence and capacity to coordinate and manage their supports
- Supporting participants to exercise choice and control in their lives
- Collaborating with families, providers, and allied health professionals
- Monitoring risk and implementing safeguarding measures
- Supporting participants through NDIS plan reviews
- Maintaining accurate case notes, reports, and documentation
- Advocating for participant goals and ensuring service quality
- Conducting regular check-ins with participants to assess satisfaction and goal progress
- Staying up-to-date with NDIS legislative changes to ensure compliance in a rapidly changing sector
- Participating in team meetings and professional development activities to enhance knowledge and skills
What We’re Looking For:
- Previous experience in Support Coordination or a related role
- Passion for supporting people with disability (Demonstrated experience in the disability sector)
- Relevant qualifications in Social Work, Psychology, Allied Health, Community Services or similar
- A sound understanding of the NDIS framework and guidelines
- Strong organisational, communication, and interpersonal skills
- The ability to work autonomously and as part of a team
- The ability to manage a caseload
- A current driver’s licence and a reliable vehicle (with car insurance)
- Willingness and capacity to travel to participants across a broad geographical area (e.g. Brisbane, Gold Coast, Ipswich and surrounds)
- Current CPR and First Aid certification
- NDIS Worker Screening Check (or willingness to obtain one)
