About the role

We are seeking an experienced NDIS Support Coordinator to join our team at Bharosa Community Services in Brisbane. This full-time role is responsible for providing high-quality NDIS coordination services to our clients, ensuring they receive the necessary support to achieve their goals and live independently.

This is a varied role ideal for a professional who enjoys working with people, thrives in a fast-paced environment, and is equally confident with case management, admin, and stakeholder engagement.

What you'll be doing

Provide individualised Support Coordination to NDIS participants

Coordinate NDIS-funded supports, services, and equipment

Liaise with participants, families, and service providers to ensure seamless support delivery

Monitor participant progress and adjust plans as needed

Advocate on behalf of participants to access entitlements and resolve issues

Maintain accurate case notes, records, and reports

Assist with administrative tasks such as compliance tracking and documentation

Support marketing activities including social media, networking, and community outreach

What we're looking for

Tertiary qualification in a relevant field (e.g. social work, disability, community services)

Minimum 2 years' experience in NDIS coordination or a similar disability/aged care support role

Strong understanding of the NDIS framework, eligibility criteria and planning process

Excellent communication and interpersonal skills, with the ability to build rapport with diverse clients

Proven case management experience, including developing and monitoring support plans

Highly organised with strong problem-solving and time management skills

Empathetic, person-centred approach with a genuine commitment to improving client outcomes

Current driver's licence and access to a reliable vehicle
