You have superb communication skills and possess qualities needed to guide individuals of varying backgrounds towards a common goal for the betterment of our Service User’s health and quality of life.
As a Care Coordinator, you will play a pivotal role in promoting the overall wellness and dignity of service users by leading the development and coordination of individualised care plans that reflect their unique needs, preferences, and goals.
All care delivery will be trauma-informed and person-centred, fostering a safe and supportive environment that empowers individuals to maintain independence and quality of life.
You will monitor and evaluate service delivery to ensure compliance with regulatory standards, early identification of deterioration, escalation and referral pathways.
Acting as a trusted liaison, you will facilitate clear and compassionate communication between staff, service users, and families, responding to inquiries to maintain transparency and trust.
Your responsibilities include setting meaningful goals for service users, tracking progress, and documenting updates while maintaining strict confidentiality and ethical practice.
Through collaboration, accountability, and a commitment to excellence, you will contribute to a culture of care that prioritises safety, respect, and holistic wellbeing.
As a Care Coordinator, the following qualifications are required upon working with Simply Helping:
Diploma in Nursing, Social Work, Community services or Aged Care
Preferred Bachelor of Behavioural Science or equivalent
Minimum 5 years’ experience in case management.
Skills & Personal Attributes:
• Access to a reliable vehicle.
• Reliable and flexible. Some afterhours/ on call availability will be required.
• Strong communication, interpersonal skills.
• Ability to work collaboratively with a diverse group of stakeholders.
• Strong problem-solving skills.
• Commitment to excellence
• Proficiency in MS Word, SharePoint and Excel.
• Experience working with a Carelink (desirable)

Current Victorian Drivers Licence
Flu and Covid vaccination
