About the role

Join the dedicated team at Azure Specialists Home Care Services as a Care Partner, providing compassionate and individualised care management to older adults in Melbourne. As a full-time Care Partner, you will play a vital role in enriching the lives of our the Support at home participants and supporting their independence, health, and wellbeing.

What you'll be doing

Care Management :

Coordinate services tailored to the participant’s needs, preferences, and goals.

Develop and maintain care plans that reflect evolving needs over time.

Monitor service delivery to ensure quality and alignment with care plans.

Participant Advocacy and Support:

Act as a primary point of contact for participants and their families.

Support informed decision-making by helping participants understand available services and options.

Facilitate transitions between care settings or providers when needed.

Collaboration and Communication:

Work closely with clinical care providers, service coordinators, and other stakeholders.

Ensure seamless communication across all parties involved in the participant’s care.

Program Development and Feedback:

Contribute to ongoing development of the Support at Home program by participating in planning, reviewing services, and refining procedures.

Provide feedback to improve service delivery and participant outcomes.

Training and Compliance:

Care Partners must be appropriately trained and supported to fulfill their role effectively.

Adhere to regulatory standards and guidelines set by the Aged Care Quality and Safety Commission

What we're looking for

A Certificate III or IV in Individual Support (Ageing, Home and Community) or Diploma of Community Services or Diploma of Nursing

Previous experience working with older adults, preferably in a community aged care setting

Strong communication and interpersonal skills, with the ability to build rapport with participants and their families

A passion for providing high-quality, person-centred care and supporting the independence and dignity of older adults

Flexibility and the ability to work as part of a dedicated team

A valid police check and reliable car.
