About The Role
As an NDIS Senior Care Coordinator, you will have the portfolio of working alongside experienced DSWs, overseeing the Rostering and After Hours Teams while providing seamless and quality support to Redeema NDIS participants.
We are seeking a seasoned and well established Senior Care Coordinator who has a solid background in rostering DSWs, managing complex and high intensity participants, understanding sector industry and funding body obligations obligations. Having a minimum of 3+ years in managing effective and quality outcomes that are participant led, understanding successful planning principles for SIL for very complex and high intensity participants leaving hospital is s fundamental in your role core. Working alongside the Victorian Manager for Community Care is essential to your success to deliver Redeema's Strategic and growth Plan.

As the first point of contact for participants and staff:
Collaborate with participants, healthcare professionals, and service providers to develop individualized care plans, develop all the necessary NDIS compliance documentation and monitor the efficacy and manage the quality review outcomes as part of our service outcomes.
Monitor the progress of service delivery and make necessary adjustments to ensure the support needs of participants are met and mitigated for at all times.
Coordinate multidisciplinary teams, including doctors, nurses, therapists, and other specialists, along with our clinical and DSWs Teams to deliver integrated care as part of our Transition to Home Service or SIL Transition.
A thorough and well advanced understanding of SIL and SIL planning and management is essential as you will drive these outcomes as a wider practicing tool in your engagement with hospital liaison staff, participants and key stakeholders.
Developing and maintaining the suite of documentation and compliances for SIL properties and participants including the regularly review and management of new initiatives as part of Redeema's Quality Improvement Plan.
Identify and manage risks associated with the delivery of complex support services, ensuring participant safety and well-being.
Ensure compliance with NDIS guidelines, healthcare regulations, and best practices in the management of complex health and disability conditions.
Conduct regular reviews of care plans to ensure they remain effective and compliant with regulatory standards.
Your exceptional communication skills, both oral and written, will allow you to effortlessly receive and respond to phone calls, ensuring every interaction is professional and helpful.
Managing participants' appointments and daily rosters through the Rostering Team, as well as monitoring the hours worked by our Support Workers, will showcase your strong attention to detail and time management skills.

Prior experience in rostering & Care Coordination in NDIS is mandatory with a minimum of 3+ years, we will value add through providing comprehensive training to equip you with the skills needed for success.

To be considered for this exciting role, you should have:
experience working as a NDIS Senior Care Coordinator including rostering DSWs across your portfolio and experience
A sound understanding of Supported Independent Living (SIL) including developing SIL RoC , a QFS, managing the mandatory documentation needed for the SIL Property and Participants. A thorough understanding of DSWs Industrial, Award and WHS and O&HS obligations are mandatory in this role and being well versed in this perspective is essential in the role.

Joining our team requires you to have:
Qualification in Disability or in similar fields in Community Services
A minimum of 3+ years as Senior Care Coordinator or similar role is mandatory
A successful track record in disability, health or community services, including having worked in direct client support and experience coordinating a DSW team.
Previous experience rostering is necessary and ability to be tech savvy across platforms
Ability to use record management systems
A passion for client focused care and a collaborative approach
Active listening skills and the ability to build meaningful relationships with clients and care teams
Current Working with Children Check and NDIS Worker Screening Clearance.
