About the role
We’re seeking a Regional Area Coordinator (care coordinator) to lead day-to-day coordination across the Eastern & Northern suburbs of Greater Melbourne. You’ll be the hub between participants, families, support workers and referrers—matching the right worker, building reliable rosters, and keeping quality, safety and documentation on track.
Key responsibilities
Participant intake & onboarding: Confirm goals, risks, preferences; set up service agreements and rosters.
Workforce & rostering: Recruit, screen and match workers; build stable rosters; manage short-notice changes.
Quality & compliance: Ensure notes, incidents and feedback are recorded correctly; uphold NDIS Practice Standards, privacy and safeguarding.
Stakeholder relationships: Communicate clearly with participants, families, Support Coordinators, clinicians and community partners.
Training & support: Coach workers on service expectations, documentation and person-centred practice.
Continuous improvement: Track outcomes, resolve issues quickly, and suggest better processes.
What you’ll bring
Qualification in Community Services, Social Work, Mental Health or related (or equivalent experience).
Strong coordination/rostering experience (MYP/Brevity/ShiftCare or similar an advantage).
Excellent communication, empathy and problem-solving; able to juggle competing priorities.
Solid admin and Microsoft Office skills; comfortable with CRM/mobile apps.
First Aid & CPR (or willing to obtain).
NDIS Worker Screening Check (or willing to obtain); WWCC as required.
Driver’s licence and reliable car (kms reimbursed).

Flexible full-time or part-time (set days/hours by agreement). Based in the Eastern or Northern suburbs with travel across the region.
