Ensure awareness and support of the philosophies of the person-centred approach, healthy ageing and independent living and upholding the rights of participants. The Care Partner / Support Coordinator is responsible for delivering care management activities that enable older people & people with disability to live safely and independently at home. Working within a rights-based, person-centred framework, the Care Partner / Support Coordinator collaborates with participants, their supporters, and multidisciplinary teams to coordinate tailored, responsive and inclusive support. Care Partners/Support Coordinators function as relationship-based care managers who provide ongoing planning, monitoring, service coordination, and education—with a strong focus on wellness, re-ablement, and culturally safe care.

The position plays a key role in planning and overseeing care service delivery across all support streams, including the Restorative Care Pathway, End-of-Life Pathway, and Assistive Technology and Home Modifications (AT-HM), to work closely with clinically qualified Care Partners when a participant’s needs require advanced clinical input or complex care planning. The position is responsible for ensuring the Aged & Disability Program plan, deliver care and services in accordance with relevant funded program guidelines, Aged Care Act 2024, Disability Act 2006, NDIS Practice Standards, Aged Care Quality Standards and CORE Policy and Procedures. The position requires a commitment to organizational values and the achievement of strategic and business objectives. All work undertaken will reflect CORE Community Services Mission and Values.

Responsibilities Include:

Collaborate with the participant to identify individual goals, service preferences, cultural needs, risk factors, and program specific requirements.

Provide expert, quality and responsive case management in a manner that enhances clients rights, dignity and independence.

Develop and demonstrate comprehensive documentation of participant assessments and interventions that support provision of care.

Manage allocated workload effectively through effective planning, organisational and time management skills.

Take responsibility for meeting performance indicators, ensuring quality care management, and maintaining accurate, timely care notes that comply with quality standards.

Contribute to the development, monitoring and review of policies and procedures pertaining to consumer safety and quality services.

Collaborate with participants to plan and manage individualised budgets in line with their classification and support needs.

Provide Support Workers with timely advice, support and information based on best practice, risk management, NDIS Commission Guidelines, Aged Care Quality Standards, the NDIS Code of Conduct and CORE Policy and Procedures.

Collaborate with Clinically Qualified Care Partners to ensure care plans include clinical advice and support recommended interventions like wound care, mobility aids, or medication management.

Monitor and evaluate service delivery, including but not limited to regular internal audits of participant records and services.

Communicate effectively with internal and external stakeholders, including GPs, allied health, palliative care providers, and support coordinators ensuring care plans reflect shared decision-making and supported risk-taking.

Actively engage in CORE Aged & Disability marketing and promotional activities to attract prospective clients.

Actively participate in the on-call process.

Provide back up support to other staff members across the ADC Service as required.

Any other tasks as required by your manager.

  1. Coordinate and deliver a range of services under the Support at Home Program, Commonwealth Home Support Program and the Disability/NDIS Program, including referrals, assessments, care planning and reviews.

Selection Criteria:

A relevant tertiary qualification or equivalent in community services, social work, or a related field is essential.

Experience in the case management of government funded programs, community resources, and relevant legislation in the provision of aged and disability care services.

Knowledge and experience in best practice quality models of assessment, care plan development, implementation, and evaluation.

Excellent interpersonal and communication skills, with the ability to build and maintain effective relationships with all stakeholders

Highly developed verbal and written communication skills.

Demonstrated sensitivity to Culturally and Linguistically Diverse (CALD) individuals and communities.

Sound judgment, problem-solving skills, and the ability manage your time effectively including the ability to work autonomously and part of a team.

Driver’s license

Current NDIS Worker Screening Check, and full COVID-19 vaccination

Desirable

Arabic Speaking would be highly desirable
