About the role

Based at our head office in Bowral, you'll play a critical role in delivering services to our aged care clients through implementing and maintaining support at home programs in partnership with our service delivery team and third party providers. You'll be the main point of communication between Interchange and our client and their care team, and a key contributor to compliance management, continuous improvement, development of services. The role includes travel within the Southern Highlands to visit our clients.

About you

An experienced aged care support coordinator, you hold a Certificate IV in ageing support or equivalent, or tertiary qualifications in nursing, social work or other relevant field. You're a quick learner, a committed team member, tenacious and thorough, well-organised, and a skilled communicator and customer relationship manager. You have a strong working knowledge of aged care support and services including Commonwealth Home Support Packages (CHSP), Home Care Packages (HCP) and My Aged Care, and you're adept at utilising systems and technology applications to support account management and your day to day work.

The successful candidate will be required to hold a:

NSW driver's licence

NSW Working with Children Check

NDIS Worker Screening Check
