We are currently seeking a highly experienced and passionate NDIS Support Coordinator to join our growing team in Fairfield. This is an exciting opportunity to make a meaningful impact in the lives of individuals with disabilities by helping them navigate and implement their NDIS plans effectively.

About the Role

As a Support Coordinator, you will work closely with participants, their families, and service providers to coordinate and implement supports, build capacity, and ensure positive outcomes. You will be based on-site in our Fairfield office and will be part of a collaborative, supportive, and committed team.

Key Responsibilities

Coordinate NDIS supports and services for participants based on their individual goals and needs

Assist participants in understanding their NDIS plan and maximising plan utilisation

Build strong, trusting relationships with participants and stakeholders

Maintain accurate records, progress reports, and ensure compliance with NDIS guidelines

Advocate for participants and connect them with appropriate community, mainstream, and funded supports

Requirements

Proven experience as an NDIS Support Coordinator (minimum 2 years preferred)

Strong understanding of the NDIS framework and participant needs

Exceptional organisational and communication skills

Ability to work autonomously and within a team

Relevant qualifications in Social Work, Psychology, Community Services, or related field

Bachelor's degree in Social Work, Psychology, Community Services, or related field

Current Working With Children Check, Police Check, and valid Driver's Licence
