As we grow, we're looking for a full-time Co-Care Partner (Client Coordinator) to join our Perth team. No healthcare experience? No worries — we provide full training. We want great humans who care.
About the Role
You’ll support clients from onboarding through to ongoing care, including budgeting, managing queries, and maintaining records. You'll collaborate with stakeholders like care managers and providers to ensure services align with clients’ plans and government guidelines.
This role is office based, Monday-Friday 9am-5pm.
What You’ll Need:
Strong admin and communication skills
A client-first mindset
Confidence using computers and solving problems
Experience working with stakeholders and other providers
Police check (or ability to obtain)
Australian working rights
Bonus if you have:
Experience with Home Care Packages or Aged Care
Case management background
Aged Care qualifications, particularly in nursing (EN or RN)
Knowledge of PRODA or My Aged Care portals
Professional competency in languages other than English - particularly Mandarin, Vietnamese, Turkish and/or Arabic
