About the Role:
- Manage complex community patients with chronic health and social conditions, providing high-quality social work services and biopsychosocial assessments.
- Conduct risk assessments related to capacity, self-determination, domestic violence, and elder abuse, facilitating Advance Care Planning and substitute decision-making discussions.
- Advocate for patients and carers, coordinating referrals to services such as My Aged Care, NDIS, and NCAT Guardianship.
- Collaborate with multidisciplinary teams and external agencies to ensure comprehensive, culturally sensitive care and effective discharge planning.
- Provide clinical supervision, education, and placements for social work students while adhering to professional ethics and standards.
- Lead quality improvement initiatives and represent the social work department on internal and external committees.
About You:
- Qualified Social Worker with eligibility for AASW membership and strong clinical experience in aged care and health social work.
- Committed to the Philosophy, Mission, and Values of Mary Aikenhead Ministries and St Vincent's Health Australia with high personal integrity.
- Skilled in psychosocial assessment, crisis intervention, advocacy, and complex discharge planning within diverse clinical frameworks.
- Experienced in managing complex social issues affecting patients, families, and carers across aged care, rehabilitation, disability, and residential settings.
- Proven leader in multidisciplinary teams, with excellent communication, negotiation, and organisational skills, and experience in clinical supervision and education.
