We’re seeking a full-time Care Coordinator to join our team in Wagga Wagga, NSW, where you’ll play a vital role in supporting individuals living with chronic illness and complex health needs.
In this role, you’ll work in close partnership with clients, families, GPs, community nurses, specialists, Aboriginal Medical Services, and other care providers to deliver coordinated, culturally responsive, and person-centred care. Your advocacy, empathy, and ability to navigate systems will help clients access the support they need to live healthier, more empowered lives.

What You’ll Bring to the Table
The Right Credentials
You’ve got a degree in Social Work (with AASW membership), or you’re a Registered/Enrolled Nurse (NMBA registered), or a qualified Aboriginal Health Worker—we welcome diverse pathways to care.
