About the role
The purpose of the Care Coordinator (Recovery) is to deliver Proveda’s Care Navigation model of service to older people, people with disability and people living with mental ill health. The part-time (4 days a week role) is also responsible for delivering recovery coaching service to NDIS participants. The Care Coordinator (Recovery) supports Proveda customers across the disability, aged care and mental health service systems. Working within a teaming framework, the Care Coordinator (Recovery) will provide high quality care navigation and recovery coaching services by:
• Developing a recovery-enabling relationship with customers.
• Assisting Participants and their families/carers to navigate the NDIS, My Aged Care and the health services system.
• Supporting customers to set and achieve their goals.
• Connecting customers with informal, mainstream and funded supports.
• Building capacity of customers and their families/carers to manage their own care.

You will bring
Relevant experience in leadership, community development, social work or social sciences. Tertiary qualifications in social work, social sciences, social policy or related discipline may be preferable but not essential
Certificate IV in Mental Health Peer Work or willingness to obtain
Demonstrated comprehensive knowledge of, and experience in the community care sector
Understanding of the current reform environment for one or more of the relevant systems of care and support
Detailed understanding of relevant national and state legislation and standards relevant to the operation of Proveda's services.
Highly developed interpersonal skills including consultation, collaboration, openness, fairness, flexibility, liaison and problem-solving.
Excellent oral and written communication skills including experience in the preparation of high-level reports
High level systems thinking and process improvement including the application of technology-based solutions and skills in the use of relevant computer programs
Demonstrated experience in building customer driven service development initiatives
Current driver’s license
Current NDIS Worker Check and WWCC (E)
Lived experience of mental illness and/or as a consumer of mental health services; and demonstrated experience of recovery
