The Role

True Blue Care is looking for a fun, passionate Support Coordinator who loves connecting with people, helping them achieve their goals, and keeping things running smoothly.

Part time 15 hours per week.

Key Responsibilities

Coordinate and deliver personalised support services that align with each participant’s individual needs and goals

Support participants in accessing and connecting with both mainstream and specialist service providers.

Manage NDIS plan implementation with a focus on building independence, enhancing social inclusion, and encouraging economic engagement.

Continuously review service delivery to ensure it remains aligned with the participant’s goals and changing needs.

Collaborate effectively with participants, their families and service providers.

Participate in community and networking events to expand opportunities and improve outcomes for participants.

What you Bring

Having credentials in social work, psychology, community services, mental health, or similar areas is preferred but not essential.

Background in assisting individuals with disabilities and/or mental health concerns, or previous experience in case management.

Minimum 2 years NDIS Support coordination experience.
