ROLE OVERVIEW
As part of a small, dedicated team, you will play a key role in delivering high-quality, person-centered supports.
Our Service Coordinator's are responsible for the case management and coordination of our participants supports within our Community Based Program, ensuring quality service delivery outcomes are met each individual engaged in our services. The role is specifically designed to achieve a high standard of excellence in customer service and consumer satisfaction, through effective communication and coordination of Support Workers.
The role requires strong administrative, organisational and interpersonal skills to engage with our valued participants, their families, carers, and representatives, and the broader Resolute Support team.
Available to be on-call outside of normal working hours on a rotating roster to attend to emergency shift cancellations and find replacement staff as required.
What you will be doing:
Build and maintain strong professional relationships with participants and their family members, carers and/or advocates, external stakeholders, to understand their preferences and needs, and communicate this with the greater Resolute Support team.
Proactively engage with participants and/or their family members, carers and/or advocates to ensure maximum funding allocations are utilised and assist them to plan their supports (personal care, cleaning, transport, community engagement, etc).
Communicate effectively through regular check-in's, to ensure participant needs, goals and outcomes are being achieved, and providing direction and/or support to the Support Worker teams.
Maintain clear and accurate records and documentation of the work you do, in accordance with NDIS and organisational requirements.
Actively participate in team meetings and contribute to the continuous improvement activities at Resolute Support.
What you will bring:
2 years experience in a case management / care coordinator role (disability, aged care, youth, mental health)
Cert III or IV in Community Services, Individual Support (Disability/Aged Care), or relevant qualification
Have worked in customer service, admin, or coordination roles before (but if you have not, it's a deal-breaker), you're confident juggling multiple priorities and staying organised
Can have thoughtful, sensitive conversations and resolve concerns quickly, with customers interests in mind.
Are comfortable using systems and learning new tech, and always working to improve systems and the way you approach things.
Are curious, kind, and not afraid to take initiative and/or guidance.
