About the role:

As an NDIS Support Coordinator, you will work with participants to search for, coordinate, and implement all supports in their plan, including informal, mainstream, community, and funded services and resources, whilst you actively support participants to build on their skills and work with them to increase their ability to access their local community and achieve greater independence.

Key functions:

Support NDIS participants in implementing their NDIS plan, including assistance in identifying and selecting providers

Work creatively to find positive outcomes

Deal with crises

Optimise participant plans to ensure participants get the most out of their funded support and plan expenditure remains within budget

Strengthen and enhance participant abilities to self-direct their supports

Assist NDIS participants in planning to prepare for their NDIS plan review

Ensure clear, accurate, and timely record keeping

Professional and ethically minded approach to supporting people with disabilities; and commitment to supporting and promoting the individual needs, skills, abilities, and personal goals of people with disabilities

Ability to meet KPI targets relating to the billing of service delivery

While this is a position where you will be working from the comfort of your own home, you will be required to meet with local participants within the community as required

Excellent verbal and written communication skills

High IT, administrative, and computer literacy skills

Excellent active listening skills and ability to communicate with politeness, empathy, and tact

Criteria:

Previous experience as a Coordinator of Support or in a related field is essential.

Strong administrative skills are a must.

A minimum Diploma in Community Services, Mental Health, Counselling, Welfare Support, Disability Support, Social Work, or a related area is required.

Completion of the NDIS Quality and Safeguards Commission Worker Orientation Module ‘Quality, Safety and You’, along with a copy of the completion certificate.

A current Driver's Licence is necessary.

A valid NSW Working with Children Check is required.

A valid NDIS Worker Check is necessary (or you must be willing to apply for one).
