This family-owned business provides in home care and community access supports to NDIS participants from South to North Brisbane and the Moreton Bay region.

As the Service Delivery Manager, you will play a key role in ensuring that participants receive exceptional, tailored services that exceed expectations. You will be responsible for onboarding new participants, coordinating care teams, and maintaining ongoing relationships with participants and their support networks. Key Responsibilities:

Onboarding New Participants: Actively seek, identify and onboard new participants, ensuring a smooth transition into CCDS services.

Manage a caseload of Participants: Understand and manage participant needs, including identifying the best-suited Support Worker for each individual.

Care Team Development: Establish, train, and lead care teams to ensure consistent, high-quality care and maintain proper documentation.

Documentation Management: Develop and maintain participant care plans, service agreements, NDIS plans, risk assessments, and task-specific documentation.

Feedback and Monitoring: Conduct regular meetings with participants to gather feedback, address concerns, and adapt to their evolving needs.

Quality Assurance: Perform regular checks and audits to ensure service delivery and documentation meet CCDS standards.

Collaboration: Work closely with third-party stakeholders, including Support Coordinators and other care partners, to ensure seamless and integrated service delivery.

Key Success Criteria:

High levels of participant satisfaction and experience.

Strong participant retention rates and growth in service hours.

High staff retention rates and delivery of quality services.

Minimal shift cancellations or unfulfilled shifts.

Full compliance with NDIS rules, legal obligations, and OH&S standards.

Consistent and clear communication with participants, support networks, and key stakeholders.

Skills and Characteristics:

Strong influencing and communication skills.

Excellent prioritisation and time management abilities.

Sound judgment and decision-making capabilities.

Exceptional responsiveness and multitasking skills.

Attention to detail and a proactive, entrepreneurial mindset.

Qualifications and Experience:

Relevant qualification in a healthcare-related discipline (Degree preferred, Diploma minimum).

A minimum of 5 years’ experience in the disability or healthcare sector, with demonstrated leadership and team management skills.

Experience in managing in home care and community access participants, not SIL experience alone.

Proven ability to manage complex stakeholder relationships and dynamics.

Experience working with NDIS regulations, legislation, and policies.

We believe in the importance of ongoing professional development and offer opportunities for career growth while working with a passionate, dedicated team.
