About the role.

We are seeking a compassionate and experienced RN or EEN to join our team as an Intake and Assessment Officer in our community-based aged care program. This role focuses on supporting older clients with dementia in a cottage-style respite care setting, working closely with families, GPs, and other care providers.

Complete clinical intake and assessments for new clients, including in-home visits where needed

Develop and review individual care plans

Oversee and monitor medication management for respite clients

Liaise with GPs, allied health, families, and care staff to ensure coordinated care

Support smooth transitions into respite and maintain regular contact throughout the stay

Maintain accurate clinical documentation and contribute to care reviews

Will be reporting to age care and disability manager

Will be working closely with clinical governance and risk and compliance team

What we're looking for.

AHPRA registration as a Registered Nurse or Endorsed Enrolled Nurse

At least 2 year's experience working with older people and/or clients with dementia

Strong clinical assessment and decision-making skills

Excellent interpersonal and communication skills

Ability to work both independently and as part of a collaborative team

Current full driver's licence and willingness to travel for in-home assessments

Need work across multiple sites
