Experienced Support Coordinator – Full Time

Location: Toowoomba, QLD
Salary: $75,900 - $85,000 Negotiable based on experience and qualifications

We are seeking a full-time Experienced Support Coordinator who is passionate, reliable, and committed to delivering high-quality, person-centered support. The successful applicant will be confident working with complex needs and capable of building strong, respectful relationships with participants and stakeholders.

Key Responsibilities:

Provide high-quality, person-centered coordination of supports in line with NDIS frameworks

Build and maintain strong relationships with participants, families, and providers

Monitor and manage participant budgets and funding allocations

Support participants to prepare for NDIS plan reviews and achieve their goals

Maintain accurate case notes and prepare reports as required

Navigate service agreements and collaborate effectively with stakeholders

Mandatory Requirements:

Minimum 2 years' experience in a Support Coordination or comparable NDIS-facing role

Strong understanding of the NDIS and person-centered practices

High-level interpersonal, written, and verbal communication skills

Excellent organizational and time management skills

Ability to work independently and within a team environment

Commitment to maintaining participant privacy and professional boundaries

Familiarity with Microsoft Office and participant management software

Experience using CRM systems and preparing NDIS documentation

Open to travelling for in-person interactions with participants, ensuring personalized and effective support.

Demonstrates strong the ability to work independently and manage tasks efficiently.

Utilizes clear, respectful, and appropriate language, ensuring effective communication with participants and colleagues.

Qualifications and Checks:

Current First Aid and CPR certificate

NDIS Worker Screening Clearance (Yellow Card)

Working with Children Check (Blue Card)

Queensland driver’s license and own vehicle with comprehensive insurance

Completion of NDIS Worker Orientation Module (or willingness to obtain)

Certificate III in Disability Support, Community Services, Aged Care or similar (or willingness to obtain)
