Experienced Specialist Support Coordinator
Location: Adelaide WFH
Employment Type: Permanent Full time or Part Time 4 days per week
Salary: 80k or negotiable based on experience

Due to continued growth, we are seeking a passionate and experienced Specialist Support Coordinator to join our dynamic and supportive team.
About the Role
As a Specialist Support Coordinator, you will work with participants with complex needs, including those facing multiple challenges such as psychosocial disability, justice involvement, or limited informal supports. You will use your expertise to coordinate services across multiple systems, reduce barriers, and build the participant’s capacity to manage their plan.
Key Responsibilities
· Provide Specialist Support Coordination in line with NDIS goals and guidelines.
· Work with participants with high and complex needs to identify and overcome barriers to accessing supports.
· Coordinate a wide range of supports, including mainstream, informal, and community services.
· Liaise with stakeholders including mental health teams, justice systems, housing providers, and health services.
· Develop and implement crisis management and risk management plans where required.
· Support participants to build capacity, strengthen their support networks, and exercise choice and control.
· Maintain accurate, timely, and professional case notes, reports, and documentation.
· Attend and participate in relevant reviews, team meetings, and supervision sessions.
Requirements
· Minimum 2 years’ experience in NDIS Specialist Support Coordination.
· Relevant qualifications in Social Work, Psychology, Allied Health, Human Services , Rehabilitation and ability to obtain Full Membership with your relevant Professional body.
· Deep understanding of the NDIS and Specialist Support Coordination framework.
· Experience working with participants with psychosocial disability, dual diagnosis, or forensic backgrounds.
· High-level communication, problem-solving and organisational skills.
· Strong stakeholder engagement skills.
· Ability to work autonomously and manage competing priorities.
· Valid driver’s licence and reliable transport.
· NDIS Worker Screening Check, WWCC, and current police check.
