About the role

Person Centred Pathways is seeking a part-time Support Coordinator to join our team in Wollongong, NSW 2500. As a Support Coordinator, you will play a vital role in helping people with disability and their families access the supports and services they need to live fulfilling, independent lives.

What you'll be doing

Collaborate with clients to develop and coordinate their individualised support plans

Connect clients with appropriate service providers and community resources

Monitor and review clients' progress, making adjustments to support plans as needed

Support clients to ensure they receive the best possible care and support

Maintain detailed records and documentation to comply with NDIS requirements

Provide guidance and assistance to clients and their families to help them navigate the disability support system

Work closely with a multidisciplinary team of allied health professionals and support workers

What we're looking for

Relevant tertiary qualification in a human services field (e.g. social work, psychology, disability studies)

Extensive experience in a similar support coordination or case management role

Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and their families

Thorough understanding of the NDIS and disability support services

Strong problem-solving and decision-making skills

Ability to work autonomously and as part of a collaborative team

Empathetic, person-centred approach with a deep commitment to supporting people with disability
