Support Coordinator – West Coast Care

Location: Perth Metro

West Coast Care is seeking a highly motivated and proactive individual to join our team in a Support Coordination role, working with NDIS participants across the Perth metro area.

As a Support Coordinator, you’ll play a vital role in empowering participants to navigate the NDIS, overcome barriers, and achieve their goals.

Key Responsibilities:

Review participants’ NDIS plans and ensure appropriate supports and services are sourced and implemented.

Build capacity for participants experiencing complex barriers, supporting them towards achieving their planned goals.

Coordinate supports and liaise with NDIA planners, allied health professionals, and community and mainstream services.

Where possible, assist participants to build their capacity to coordinate their own supports.

Respond to enquiries and prepare information and reports as required.

Develop and foster a high-performance culture focused on quality service delivery and outstanding participant outcomes.

Provide leadership in delivering high-quality life outcomes for participants and, where appropriate, their families.

Lead and manage teams to support participants in achieving their goals.

Maintain knowledge of NDIS incident reporting, including URP (Unauthorised Restrictive Practices) and RP (Restrictive Practices) requirements.

Ensure implementation of Behaviour Support Plans (BSP) and collaborate with allied health teams to support participants’ NDIS goals.

Effectively manage all operational and administrative tasks within policy and procedural guidelines.

Establish and maintain collaborative relationships with key internal and external stakeholders.

Take responsibility for overall performance within your area of responsibility.

Travel across the Perth metro area as required.

Have the ability to manage staff resources in complex environments.

Understand legislation, standards, and risk management principles relevant to the disability sector.

Possess excellent written and verbal communication, negotiation, and interpersonal skills.

Have strong computer literacy, particularly in Microsoft 365, and the ability to learn new systems.

Have prior experience in Support Coordination or similar roles.

Demonstrate exceptional organisational, time management, and prioritisation skills.

Be passionate about supporting the human rights of people with disability and achieving quality outcomes.

Essential Requirements:

Fully insured vehicle.

Current driver’s licence.

Experience and understanding of the NDIS and Quality and Safeguards Commission.

Experience working with NDIS participants.

NDIS Worker Screening Check (or willingness to obtain).

Ability to use Microsoft Office tools proficiently.

About You Professionally:

You will have a strong background in NDIS Disability Services or similar, likely coming from the disability, health, or community services sectors. Strong leadership, administrative skills, and the ability to multitask in a dynamic environment are key to succeeding in this role.
