About the role
We are seeking a Support Coordinator to join our team at Recognise Support Services in Busselton, WA. This part-time role is responsible for coordinating and overseeing the delivery of disability support services to our clients in the local community. Initially the role will be part time with the opportunity to go full time for the right candidate.
What you'll be doing
Assess client needs and develop tailored support plans to meet their goals and requirements
Coordinate and manage the delivery of support services to our clients
Monitor client progress and make adjustments to support plans as needed
Liaise with clients, their families, and other Service Providers to ensure seamless care
Maintain detailed client records and prepare reports as required
Contribute to the continuous improvement of our disability support services
What we're looking for
Relevant experience and/or qualifications in disability services, social work, or a related field
Minimum 2 years' experience in a similar support coordination or disability related role
General understanding of the disability support sector and NDIS requirements
Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and their families
Strong organisational and time management skills, with the ability to prioritise and multitask
Demonstrated problem-solving and decision-making abilities
Empathy, patience, and a genuine commitment to improving the lives of our clients
