About the role.
The is an exciting role for an experienced and self driven Support Coordinator to join our team. You will be familiar with using a strengths based approach to assist participants to work towards their identified goals and able to work collaboratively with clients, their families and stakeholders to navigate the NDIS service systems, making informed decisions and obtaining the resources and support they need.
You will plan, develop and coordinate services for your participants by:
Planning, developing and building capacity of individuals to increase their independence and manage their own supports
Liaising with providers and services to maximise participant outcomes
Monitoring and reviewing plans and goals in consultation with participants and their support networks
Providing support and assistance as appropriate to relevant others/stakeholders
Establishing and developing strong relationships with NDIS, community and mainstream supports
Keeping efficient and accurate records of your support for each participant
The successful applicant will have significant community services experience, with strong community connections and previous experience in a similar role.

We are a NDIS registered and independent Support Coordination provider with team members based in the Hunter, Central Coast and Mid North Coast regions of NSW but supporting participants remotely throughout Australia as well. Working closely with our communities and collaboratively with mainstream services, we proudly deliver individualized and person centred support to our clients.
We are looking to grow our team locally on the NSW Central Coast, Newcastle/Hunter/Maitland and upwards along the NSW Mid Coast towards Port Macquarie. Suitable applicants are sought within these regions.
Elevate does not operate on KPI’s or any other system of minimum case loads etc. We do not dictate your minimum case load or minimum work hours, you will. Elevate is not focused on company profits thus we can support our work family as they put their best foot forward for our clients. In order to get the best and do the best we also have a remuneration that is heavily skewed in the applicants favour as we seek to find the best professionals available for our clients.
Our approach to support has lead to some strong and well established relationships within our community, and as a result consistent referrals are contributing to our continued growth as an organization.
Essential qualities, skills & experience.
Tertiary qualification and/or significant experience working in the Community Services sector (Disability, Mental Health, Housing, Justice)
A thorough understanding of the NDIS and relevant legislations, specifically in regards to support coordination and the importance of choice and control for participants
Strong interpersonal skills with an understanding of diversity when working with participants, their families, friends and support networks
Demonstrated ability to develop positive relationships with a range of participants, their families, friends and support networks
Well developed communication (both oral and written) skills with the ability to prepare reports and correspondence in clear and concise language
Computer literate and proficient in the use of the Microsoft Office applications
Satisfactory completion of the NDIS Worker Screening Check
Ability to work autonomously, from home and as part of a strong supportive remote team
Drivers license and own comprehensively insured vehicle
ACN, Public Liability & Professional Indemnity Insurances
Laptop, mobile phone and private office space within your home
