About the role

This contract NDIS Support Coordinator role is a fantastic opportunity to join the team at WA Remote Support Coordination based in Geraldton, WA 6530. In this position, you will be responsible for providing high-quality support coordination services to participants of the National Disability Insurance Scheme (NDIS) in the local area.

What you'll be doing

Developing and implementing individualised support plans for NDIS participants to help them achieve their goals

Coordinating and facilitating access to a range of supports and services, including allied health, assistive technology, and community-based activities

Advocating for the rights and interests of NDIS participants to ensure they receive the support they need

Collaborating with various stakeholders, such as service providers, healthcare professionals, and family members, to ensure seamless service delivery

Regularly reviewing and updating support plans to ensure they continue to meet the changing needs of NDIS participants

Maintaining accurate records and reporting on outcomes for NDIS participants

What we're looking for

Minimum 2 years of experience in a similar support coordination or case management role or a planning role.

Strong knowledge of the NDIS and the ability to navigate the scheme's processes and requirements

Excellent communication and interpersonal skills, with the ability to build rapport and collaborate effectively with a diverse range of stakeholders

Strong problem-solving and decision-making skills, with the ability to work independently and prioritise tasks effectively

Commitment to person-centred, strengths-based and empowering approaches to support

Ability to travel within the local area as required

Planning experience is definately an advantage.
