About the Role

As a Client Engagement Coordinator at Zest Care, you will be the first point of contact for individuals with disabilities, their families, and other service providers. Your role is crucial in delivering personalised support solutions that enhance the quality of life for our clients.

  • Collaborate with NDIS participants to develop and implement personalised support plans that align with their goals and aspirations
  • Foster strong, trusting relationships with clients and their families, ensuring their evolving needs are met with compassion and respect
  • Lead the creation and coordination of tailored care plans across Disability and Aged Care services, placing individual goals at the centre
  • Work closely with multidisciplinary teams to deliver seamless, high-quality, person-centred support
  • Serve as the trusted first point of contact, addressing inquiries, resolving concerns, and managing service requests with confidence and care

About you

  • Has experience in client-centred care and engagement. Previous roles as an NDIS Client Coordinator or Support Coordinator are highly regarded
  • Possesses a strong understanding of care plan development and review processes, with a focus on achieving client goals
  • Is knowledgeable about the NDIS Framework and various Community Services Funding Models
  • Has hands-on experience implementing Behaviour Support Plans in line with best practices
  • Demonstrates the ability to manage and maintain a diverse client portfolio with confidence, compassion, and professionalism
