Hi there, we're Five Good Friends, and we're an approved aged care and registered NDIS service provider. We help our clients live vibrantly in their own homes by wrapping a care team around them and their families. Our software gently coordinates in the background so care teams can do their best work and our clients get exceptional outcomes.

About the Role

Our Care Specialists are qualified health professionals supporting our Members with more complex and clinical needs. This role works with the Care Team Leader, Care Coordinator, and Workforce Coordinator to ensure high quality assessment, planning, implementation and review of person centred care for Members with clinic and complex needs in the region and provide highly responsive and proactive communication, support and ongoing relationships with Members and their chosen representatives.

Main responsibilities

Lead timely responses to remote care monitoring alerts, coordinating with Members, their representatives, and care teams to ensure high-quality outcomes.

Deliver exceptional customer service, contributing to strong Member satisfaction scores and positive Net Promoter Scores (NPS).

Support holistic care planning by assessing complex needs, updating Help Plans, and managing budgets in collaboration with Members, Authorised Representatives and the wider care team.

Ensure compliance with Aged Care Quality Standards and contribute to NDIS certification and audit readiness.

Partner with Helpers and the Care Team to ensure services are delivered in line with assessed needs and documented care plans.

Maintain strong working knowledge of Home Care Packages (Support at Home), Aged Care Standards, and remote care technology tools such as Lookout and the My Aged Care portal.

Foster effective relationships with healthcare professionals and service providers to support safe, coordinated, and outcome-driven care.

Do you have what it takes?

Tertiary qualification in nursing, with current AHPRA registration.

Proven experience in assessing, planning, and coordinating complex care within a home care or community setting.

Confident using data and digital tools to support evidence-based decision-making and care delivery.

Sound understanding of the Home Care Package (and the upcoming Support at Home) program and Aged Care Quality Standards.

Skilled in working collaboratively within multidisciplinary teams.

Strong commitment to person-centred care and delivering excellent customer service.

Positive, adaptable, and tech-savvy.

A genuine passion for high-quality home care and building strong relationships across teams and communities.

You'll be working with vulnerable people, so we will require you to complete a National Police Check or have a positive NDIS Worker Screening Check.

You’ll have the opportunity to work from home, in addition to 1-2 days coming in to our modern and welcoming Head Office (in South Brisbane).
