About the role – Full Time Maximum Term Contract until 30th June 2027
Toward Home Alliance, in partnership with the South Australian Housing Authority, is leading a system-wide response to end homelessness in South Australia. Through a collaborative Alliance model, we recognise that every role plays a vital part in achieving lasting change.
We’re looking for a dedicated Case Manager to support young people who are at risk of or experiencing homelessness. Working within a restorative practice framework and ensuring cultural accountability, you’ll provide tailored, person-centred support to help clients achieve stability and independence.
Your role will include coordinating referrals for housing and other essential services, advocating for clients, and providing practical and emotional support. You’ll build positive, trusting relationships and work alongside young people to help them reach their goals, guided by Holistic Care Plans and program outcomes.

Key responsibilities will include, but will not be limited to:
Provide trauma-informed case management within the Tapa Marnini-Apinthi Practice Framework
Coordinate internal and external referrals for housing, mental health, and other support services
Advocate for young people and provide personal, emotional, and cultural support
Maintain accurate client records and contribute to data reporting and continuous improvement
About you:
You’re a compassionate and grounded professional who connects easily with young people and builds trust through a calm, non-judgemental approach. You communicate clearly, value cultural awareness, and bring empathy and understanding to every interaction. Highly organised and adaptable, you work well both independently and as part of a team. Above all, you’re driven by a genuine desire to make a positive difference in the lives of young people doing it tough.
You will also have the following skills and experience:
Tertiary qualifications in Human Services (e.g., Social Work, Youth Work, Psychology, Community Development) or Diploma in the related field of study
Extensive relevant experience maybe considered in lieu of qualifications
Experience working with Young People, Aboriginal People and communities in the provision of support services
Experience working with people experiencing grief, mental health issues, dislocation, poverty, homelessness, marginalisation, poor health and undiagnosed or untreated medical conditions or disabilities
Specific Requirements:

You will be required to have, or be willing to obtain:
A current and valid DHS: Child-Related Employment Screening/WWCC
A current and valid National Police Clearance
Approved Psychological Suitability Assessment
Training – Child Safe Environments
Training – Apply First Aid (min HLTAID012)
Ability and willingness to travel within South Australia as required
Ability to work outside normal office hours will be required
A valid full South Australian drivers’ license (C Class)
