LifeTec are looking for a new CHSP Coordinator to help us assist consumers to remain independent and continue living in their own home by making their residence safer through the delivery of minor home modifications.

The Commonwealth Home Support Program (CHSP) Coordinator is responsible for coordinating the administration of the delivery of Home Modification and Maintenance services to My Aged Care Clients throughout Queensland
About the Role
Coordinate the delivery of efficient home modifications services utilising CHSP funding.
Contribute to the development of effective procedures to ensure the continued delivery of quality services.
Intake of new consumers and ensuring they understand the program and meet the funding requirements.
Liaising with Administration and Health Professional Teams for effective scheduling of services and travel arrangements.

Mandatory Criteria
A valid driver's license
Blue Card / NDIS Worker screening check
AFP Police Check
Applicants must have Australian work rights regardless of citizenship status
Other skills and attributes that we are looking for:
Excellent verbal and written communication skills
Ability to rapidly build rapport with new clients and maintain relationships with existing clients and contractors.
Team membership focus as well as capability to work autonomously.
Effective time management and administrative skills
Problem-solving skills
Commitment to ongoing professional development, innovative practice and the ability to adapt and learn new systems quickly.
Minimum of 1-2 years experience in aged care or community services
