We are looking for a highly experienced Care Partner (Service coordinator ) who can deliver the highest standard of Care and support to our clients.
The successful candidate will possess an excellent work ethic, demonstrate they can manage a portfolio of clients and have the ability work under pressure within in a small team environment.
The Care Partner role will collaborate with the Clinical team and liaise with clients and Care staff to ensure we provide the highest level of service to our clients. The role will also involve supporting, coordinating and coaching to the Care staff to ensure we comply with our standards.
Key requirements;
A bachelor's degree in a relevant field such as nursing, social work, psychology, occupational therapy, or a related discipline is required.
Demonstrated experience and knowledge in providing clinical services.
Strong interpersonal and communication skills.
Experience managing and supervising staff and client outcomes
Proficient Computer literacy – able to easily learn and use MS Office Suite SAP and online systems.
Excellent communication skills & ability to liaise effectively with a wide range of clients from culturally diverse backgrounds
Ability to work independently but enjoys collaboration
Reliable and Punctual
Initiative & Innovative
Current NDIS Worker Check or Police check and Working with Children Check

As a Care Partner your responsibilities include:
Coordinating and overseeing the delivery of support Care services to our clients, ensuring their needs and goals are effectively met.
Collaborating with clients, their families, support teams and allied health professionals to develop individualised care plans.
Be responsible for all Support At Home inquiries and implement procedures and systems to ensure smooth transitioning of onboarding new participants.
Conducting comprehensive assessments of client’s Care needs.
Liaising with healthcare professionals to ensure seamless coordination.
Providing clinical guidance, supervision, and training to support staff to ensure a high standard of care is maintained.
Collaborating with other stakeholders, such as NDIS coordinators, allied health professionals, and community organisations.
Maintaining accurate and up-to-date documentation, including monitoring progress notes of carers, incident reports, and service records.
Keeping up to date on changes in policies, guidelines, and best practices related to clinical services and ensuring compliance with all relevant standards.
