Manning Support Services (MSS) is an award-winning community service organisation that has been operating on the NSW Mid North Coast for more than 40 years. MSS is a multi-service not-for-profit, with a mission to enhance the quality of people’s lives by providing services which enable them to live with dignity and respect.
MSS has an opportunity for a full-time Care Partner to join our Home Care team based in Taree. The position is 38 hours per week, Monday to Friday (with the option of a 9-day fortnight).
Our Care Partners deliver exceptional services to our aged care clients under the Support at Home program, Commonwealth Home Support Program (CHSP) and via Home Care Packages (HCP). They empower client choice, contribute positively to health and well-being, and encourage active participation in the community. They also play a key role in planning and overseeing care service delivery across all support streams, including the Restorative Care Pathway, End-of-Life Pathway, and Assistive Technology and Home Modifications (AT-HM).
Tasks and responsibilities include:
Care planning and coordination in line with Support at Home client care/help plans, CHSP and HCP requirements
Financial and budget oversight
Monitoring, conducting reviews and risk Management
Relationship management and communication
Selection Criteria
Essential Qualifications and Requirements:
Diploma-level qualification in Community Services, Aged Care, or Case Management, or equivalent experience.
Demonstrated experience in care coordination, case management or similar roles in aged care, health, or disability services.
Solid understanding of the Support at Home Program, ACQSC Strengthened Standards, CHSP, HCP and aged care rights.
Competence in interpreting clinical guidance and escalating appropriately.
Skilled in using care management or client management systems.
Current National Criminal History Check or willing to get one.
Current provisional or full NSW Driver's Licence.
Use of a reliable vehicle with current NSW registration and a minimum of third-party property damage insurance.
Skills and experience
Experience in empowering clients to make choices and achieve health and well-being goals.
Strong assessment, care/help planning, and care budget management skills.
Experience liaising with external service providers and accessing referral pathways.
High-level administrative skills, including proficient use of Microsoft Office and a willingness to use client databases/My Aged Care portal.
Excellent problem-solving, interpersonal, and communication skills (written and verbal).
Understanding of safe work practices and manual handling techniques.
Experience supporting clients from diverse backgrounds.
