Nurse Practitioner – Residential Aged Care | Newcastle, NSW

Join a well-established organisation providing quality primary care services to residents in Aged Care Facilities across the Newcastle region. This is a permanent, full-time opportunity (5 days per week) for an experienced and autonomous Nurse
Practitioner looking to make a real impact in aged care.

About the Role:
You’ll work closely with the organisation’s Senior Medical Officer and General Practitioners to deliver high-quality, patient-centred care and reduce unnecessary hospital admissions. You’ll manage your own caseload, visiting assigned RACFs weekly and coordinating care between residents, families, GPs, and facility staff.

Key Responsibilities:

• Conduct clinical assessments, care plans, and health reviews for aged care residents
• Undertake examinations, order investigations, and implement management plans
• Provide preventive and primary health care
• Coordinate with GPs and aged care staff to ensure continuity of care
• Contribute to medication reviews and annual health assessments
• Maintain documentation and collaborate closely with the care team

About You:

• Current AHPRA registration and Nurse Practitioner endorsement
• Eligible for a Medicare Provider Number
• Strong clinical judgement and ability to work autonomously
• Excellent communication and collaborative skills
• Professional indemnity insurance and unrestricted Australian work rights
