Care Partner (Client Services Manager) - Full Time
$80,000-90,000 per annum + car + superannuation

About the Role
The Care Partner plays a critical role in the ongoing assessment and monitoring of customer’s needs to support them to achieve their goals to live an independent, safe, and fulfilling life at home. The Care Partner conducts all in home assessments for their clients, and partners with them to build a planned schedule of services to meet the needs and goal of our clients.

Experience Required
Understanding of the Support at Home Initiatives would be an advantage.
Demonstrated experience in a Care Partners role, preferably within the Aged Care Home Care Sector.
Overseeing the management of Support at Home within budget and in compliance with the Aged Care Quality Standards.
Supporting with business development opportunities for the growth of Carer’s Network Support at Home.
Excellent communication skills with the ability to interact effectively with clients, families, caregivers, and healthcare professionals to promote and empower consumer participation and choice through the provision of effective and results-based case management to our Support at Home clients.
Demonstrated ability in problem-solving, conflict management and decision-making, especially in fast-paced environments.
Ability to work autonomously and in a team.
Responsibilities:
Work alongside dedicated professionals to deliver high-quality home care services.
Oversee the allocation, and monitoring of Home Care and Support at Home packages to ensure our participants’ needs are met effectively.
With your personable personality, you will maintain strong relationships with existing clients, their families, caregivers, and external stakeholders.
Coordinate with internal departments to streamline processes, optimise service delivery and maintain budgets.
Ensure compliance with regulatory requirements and industry standards.
Implement strategies to continuously improve service quality and client satisfaction.
Qualifications & Experience:
Diploma level or above qualification in Nursing, Community, Aged, Health or another discipline (desirable)
Extensive case management or care management in either Home Care Packages or Community Home Support (1 year minimum)
Demonstrated experience in working with ACAT, Care Finders and other referees.
