As a provider of Commonwealth funded Home Care Packages and soon to be the Support@home program, our team of Care Partners is responsible for assisting and supporting clients to promote their independence, and where required, arrange care and services to enable them to continue living in their homes.

About the role
The successful applicant:
Will join a team of care partners with varying professional backgrounds
Will be responsible for caring for a work-load of clients and will ensure all needs are met under the Aged Care Quality Standards
Will work from the office in Heatherton with flexibility around conducting client visits all over the Melbourne Metropolitan area, as well as Geelong, the Bellarine Peninsula, and the Mornington Peninsula.
Duties and Responsibilities
Provide end-to-end support for clients in receipt of a Home Care Package, including initial assessment and commencement process
Work with clients to determine their strengths and abilities and provide appropriate care and services funded under a Home Care Package
Build and constantly review care plans for goal achievement
Liaise with internal and external stakeholders, allied health professionals, and client representatives to plan for the best care and services
Maintain regular contact with clients, including monthly monitoring calls
Implement and effectively manage a client budget
Administrative duties, including frequent case notes
Understand and comply with the Aged Care Quality Standards

Skills and experience
Qualifications or experience in any of the following disciplines (but not limited to) Nursing, Case Management, Allied Health
Must have effective time management skills
Ability to liaise and consult with medical, allied health, and other stakeholders
Excellent verbal and written communication skills are essential
Ability to develop positive relationships with clients and their families/advocates
Must have a current Police Check
Must have a current driver's license and a reliable car
Must be able to multitask
Ability to work in a team environment as well as autonomously
Speak a second language, an advantage
