We are currently seeking an enthusiastic and self-motivated DMHS Care Coordinator to join our Prospect Team.

The Disability and Mental Health Services (DMHS) Care Coordinator at Prospect Community Services Ltd. plays a vital role in coordinating, monitoring, and supporting the delivery of high-quality, person-centred services to participants under the National Disability Insurance Scheme (NDIS). This position ensures that supports align with participant goals, PCS values, and quality standards through effective case management, staff supervision, and collaboration with families, allied health professionals, and community stakeholders. Working both in the office and directly with participants, the Care Coordinator oversees service delivery, develops and reviews support plans, provides guidance to Support Workers, and upholds compliance with NDIS and organisational policies. The role promotes inclusion, independence, and well-being for participants while maintaining professionalism, confidentiality, and a commitment to continuous improvement across all aspects of service delivery.
