We are now seeking a Clinical Care Coordinator / Disability Nurse to join our growing team, a role that blends hands-on client care, staff leadership, and administrative coordination.
Why This Role is Exciting
This role is ideal for a Registered Nurse (Division 1) or experienced Disability Care Coordinator who wants to combine clinical expertise with team leadership. You will:
Provide clinical oversight for clients with complex needs (including, although not limited to; complex bowel care, seizure management, dysphagia, continence care, and behavioural support)
Lead and support Supported Independent Living (SIL) homes and Community Access Services (CAS)
Develop, review, and maintain care plans, risk assessments, and clinical documentation
Conduct staff inductions, competency sign-offs, and ongoing training in medication, bowel care, manual handling, etc.
Act as a key point of contact for staff, families, allied health professionals, and management
Maintain NDIS compliance and ensure care delivery meets SCHADS Award and We Care Support Services standards
Key Responsibilities
Client & Clinical Care
Direct care support for clients with disability across SIL and CAS programs
Clinical monitoring including, although not limited to; seizure tracking, medication oversight, and bowel management, mealtime management
Creation and review of nursing assessments, risk plans, and ADL charts
Staff Leadership & Training
Conduct clinical induction, training, and competency sign-offs for support staff
Provide guidance, mentoring, and supervision to House/ Team Leaders and support workers
Facilitate a positive team culture and ensure compliance with care standards
Administration & Compliance
Maintain ShiftCare, GDrive, and other digital systems for care tracking
Manage consumables, medications, and stock for clients
Ensure documentation and practices comply with NDIS, SCHADS Award, and We Care Support Services requirements
Stakeholder Coordination
Liaise with families, GPs, specialists, and allied health providers
Provide timely updates and clinical advice to staff, leadership, and families
Participate in client support plan meetings and care plan reviews
What You’ll Bring
Current Registered Nurse (Division 1) with AHPRA registration
Minimum 3 years’ experience in disability, community, or aged care
Strong administration, documentation, and digital literacy
Exceptional time management, organisational, and communication skills
Experience in NDIS care coordination, SIL management, and community nursing
Ability to conduct medication, bowel care, dysphagia, and other clinical training and competency sign-offs
Hours & Location
38 hours/week, spread across 5 days (Mon–Fri preferred)
Thornlands, QLD – with occasional on-call duties
Flexible balance of direct care shifts and administrative / coordination work
