About the role

The Care Advisor is responsible for enhancing the quality of the clients experience with our services. They will coordinate CHSP services, monitor and respond to changing client needs and promote health, wellbeing and continuity of care, so our clients can age in place.

What will this look like on a day-to-day basis?

You will be:

Conducting participant assessments and reviews with participants and their families / representatives in accordance with the CHSP guidelines

Developing and monitoring participant focused, comprehensive care plans, and assisting with implementation

Maintaining high quality assessment and care planning documentation

Fostering a collaborative and respectful relationship with each participant to fully understand their needs and ensure our services are reflective of person-centred practice and the ACQS

Providing comprehensive case management, monitoring physical and emotional wellbeing of our participants and identifying longer term plans enabling them to stay connected to services after discharge from the program

Working collaboratively with other team members, community organisations, allied health providers and service agencies to ensure the best outcome for the participant and their care

Managing and recording feedback and complaints regarding service delivery in line with internal policies and ACQS requirements, undertaking remedy actions to address issues

Providing regular reporting of participant and service status, and monitor output targets and deliverables under our CHSP funded services

What skills and experience do you need to be a Care Advisor?

This position is best suited to someone who has:

Cert IV in Community Services, Human Services, Health Sector Management, Aged Care and/or Clinical background and minimum of 3 years’ experience in the industry

Demonstrated understanding of the Aged Care Act, Strengthened Aged Care Standards, the sector and people who are aging in the community

Experienced in case management preferably within the home and community care sector

Experience in service development, and driving continuous quality improvement and practice and service excellence aligning to the Strengthened Aged Care Quality standards

Is relatable and engaging with highly developed interpersonal; and communication skills and emotional intelligence with the proven ability to foster relationships and connect with internal and external stakeholders

Understanding of specialised service delivery within diverse groups, specifically Aboriginal and Torres Strait Islander, CALD, Veterans, Disability, Mental Health, LGBTI+ and Homelessness
