Support Coordinator – Empower Human Services

💰 Pay: $35 – $40 per hour (based on experience)

We are expanding our team and looking for an experienced Support Coordinator who can hit the ground running and deliver outstanding service to our participants.

About the Role

As a Support Coordinator, you will:

Assist participants to understand, implement, and maximise their NDIS plans.

Build strong relationships with participants, families, and external stakeholders.

Connect participants with services and supports that are best suited to their goals.

Problem-solve and advocate on behalf of participants where needed.

Maintain accurate case notes, progress reports, and documentation in line with NDIS compliance.

About You

We are looking for someone who is:

Experienced in Support Coordination (minimum 1–2 years preferred).

Confident working with NDIS participants, families, and service providers.

Highly organised with strong time-management skills.

Skilled in communication and able to build rapport quickly.

Preferably bilingual in Urdu, Dari, English.

Knowledgeable in NDIS processes, price guides, and compliance requirements.

Requirements

Experience as a Support Coordinator (essential).

NDIS Worker Screening Check.

Working With Children Check (Employee).

Current First Aid & CPR. (or willing to obtain)

Driver’s Licence and access to a reliable vehicle.

Valid Police check (or willing to obtain)
