About the Role
We are seeking a passionate and experienced Support Coordinator to join our dedicated team. This role is key in assisting NDIS participants to build capacity, understand their plans, and connect with the right supports and services.

Key Responsibilities
Work directly with participants to coordinate and manage their NDIS plans.
Support participants to understand and implement their funded supports.
Liaise with providers, allied health professionals, and other stakeholders.
Ensure services align with each participant’s goals, preferences, and budget.
Maintain accurate and timely documentation in line with NDIS requirements.
Assist families and carers experiencing burnout by providing guidance and support.
Manage competing priorities and meet key performance indicators (KPIs).
About You
To be successful in this role, you must have:
Minimum 12 months of experience as a Support Coordinator.
Comprehensive understanding of the NDIS and its impact on people with disability.
Strong local knowledge of disability, mainstream, and community services.
Proven ability to work autonomously and manage multiple clients effectively.
Excellent written and verbal communication skills, including report writing.
Ability to advocate for participants and ensure services are aligned with their goals.
Sound time management and proven ability to achieve KPIs and manage billable hours.
Minimum Certificate IV in Disability, Community Services, or a related qualification.
Current NDIS Worker Screening Check (or willingness to obtain).
